1What is a plan of care?
Your doctors and our team of home care nurses will work with you to make up your plan of care. This will outline what you will need from your services. Together you will assess:
  • What services you will need
  • A schedule that will fit your needs best
  • What types (if any) of medical equipment you will need
  • Any therapies you might need
  • Goals that you and your doctors feel can be achieved through services.
2 Why choose home care?
It is a proven fact that the speed of recovery is faster from the comforts of home.  Depending on your hours and insurance, home care could even be more affordable than a nursing home or other facility. Residing at home could also decrease the risk of infections and sicknesses transferring from patient to patient. With home care you can:
  • Recover faster
  • Reduce the chances for infections and the chances to get sick
  •  Recover from the comforts of your own home
  • Involve your family members in your recovery and care
  • Gain more independence while still getting the help you need
3What agency is right for me?
Here are some helpful questions to think about when choosing the right agency for your needs
  • Does the agency take my insurance?
  • Does the agency provide the services I need?
  • Who will supervise my services?
  • Will the agency work with my doctors?
  • Is there a back up plan for when a provider does not show up?
  • Are providers available during the hours I need?
  • How long has the agency been servicing the community?
  • Will the agency include my family members in my care plan?
  • Will my insurance cover services and supplies?
  • Do supervisors make regular home visits?
  •  Who is available to talk to regarding questions and concerns I may have?
4What is skilled nursing
We staff nurses who are available on an as need basis. They will not do providers services, however if there is a problem, or you are in need of medical attention such as adjusting medical equipment or changing bandages, they are here to help.
5Who supervises the care I receive?
We have 4 field supervisors who are in charge of areas based on zip codes. Once a supervisor is assigned to you he or she will oversee your care. They will be in charge of hiring your providers and assessing the care as needed.
6Will a supervisor be making frequent home visits?
Our supervisors will make home visits as needed to assess situations. Situations such as a post hospital visit or an orientation of a new provider will require a supervisor to stop by. There will also be an annual visit in which the supervisor will assess providers who have been providing care for you.
7What happens if my provider doesn’t show up?
If your provider is sick or just doesn’t come into work all you need to do is give us a call. Once we are aware of the situation we will begin working on getting a “floater” or “special attendant” to your house. Floaters/special attendants are providers who are on call and can go to any client if needed. We have several staffed and ready to help.
8Does your agency take my insurance?
We take the following insurances:
  • Medicare/Medicaid
  • Amerigroup
  • Molina
  • Superior
9What hours are your services available?
While our offices are only open Monday – Friday between 8:00 am and 5:00 pm, our staff is on call 24/7. Our providers are also available 24 hours a day. If your provider is unavailable we will find a temporary provider to service you.
10What services do you provide?
We provide the following services:
  • Skilled nursing
  • Personal care
  • Bathing services
  • Dressing and grooming assistance
  • Housekeeping
  • Weekly grocery shopping
  • Companionship
11How much does your agency charge for private pay?
Private pay is for those individuals who do not have insurance or whose insurance is not accepted at our location. This is out of pocket pay in which the amount would depend on the type of care and services needed. For more information you can call any of our offices.

12What type of licensing does your agency carry?
  • PAS (personal assistant services)
  • LHHS (Licensed Home Health Services)
  • LCHH (Licensed and Certified Home Health)
  • What types of payments are accepted?
  • Medicaid/Medicare
13Is a deposit or contract required?
No, while some agencies will require either a contract or deposit (or sometimes both) our agencies do not require either one for our services.
14What services does Medicare cover?
  • Skilled nursing
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Social workers
  • Home health aids
15How long do services last through Medicare?
Medicare assesses your case every 60 days. At that time, if care is still needed, they will issue another authorization. When assessing your case, they will determine if more hours are needed, and supply them accordingly. If an emergency comes up with requires you to have more hours before the 60 days is up, you can speak to your case worker provided through Medicare. They will be able to see what that can do about getting additional hours set up.
