A Registered Nurse will come out to your home, free of charge, to assess you or the patient, in order to determine the type of care you or the patient need.
We strive to accommodate each individual's unique needs, providing a holistic approach. Your Plan of Care includes your personal physician, ensuring that all physician prescribed orders are followed as directed.
As many as 24 hours a day of continuous Care giving and Nursing services for both long and short term needs based upon your individual circumstances.
We pride ourselves on the fact that our company is operated by experienced nurses. From the moment you pick up the phone to inquire about our services, you will be speaking with a nurse.

Personal Care
Personal Care is one of our most important duties. We are able to help with simple tasks like getting dressed, more difficult actions like bathing, or even total assistance with all personal care.
Our staff is oriented to your specific needs so you always have someone who is trained in the task you require. Duties include bathing (using bath, shower, or bed bath), oral hygiene, dressing, ?incontinence, mobility, following exercise regimen, or any other personal care need.
Our flat, hourly rate does not go up based on the amount of personal care a person may require.
Transportation
Driving is a symbol of freedom. When we loose the ability to drive on our own this doesn't mean we can't get out and do the things we once did.
With a caregiver from Axiom Home Health behind the wheel, seniors are once again able to get out and do the things that give them independence. Consider us your private chauffer driving to doctor appointments, the store for some shopping, or maybe just to get out on the road to enjoy the view.
If you have the car we have the drivers.
Meal Preparation
Meal preparation is very important in maintaining health. Many clients are on specific diets and need to have meals prepared to meet those needs.
Our staff doesn't just make whatever they feel like making that day. You tell us what you want! It's like having your own personal chef! Want a steak? Just tell us how you want your steak prepared!
We also prepare meals for diabetic diets, low sodium diets, low sugar diets and many other special diets.
Medication Reminders
As a senior ages, the amounts of medications he or she is asked to take often goes up. Sometimes the sheer volume of pills can confuse people. Add to this any age-related memory issues and it is a recipe for disaster. The United States Department of Health and Human resources estimates that only 50 to 60 of patients are adherent to taking their medications as prescribed. The cost of non-adherence is estimated to be $100 billion, and non-adherence causes 10 percent of all hospitalizations.
Our caregivers can assist in medication management through tracking of medication times, dosages and types to be taken. We conduct a complete inventory of medications upon admission, which we constantly update as needed.
Companionship
It is our belief that no senior should feel left alone. Axiom Home Health can provide a needed companion for a senior who lives alone, or is alone for portions of the day. During this time we can spend time reading, sharing stories and provide much needed social interaction.
Respite Care
Caring for a sick or disabled loved one can require full-time attention. Families and spouses often need time to themselves to regroup or to tend to other needs – such as running errands or attending to personal business.
Axiom Home Health can provide care while you take time to tend to your needs. Whether it be a vacation or a few hours of shopping, we can be there for you and your loved one.
Errand Service
With Axiom Home Health you can start doing those things you stopped. Go to your weekly bridge game, visit with some old friends, or do whatever you want to do.
Our service is great for people who take care of a loved one on their own. It gives you a chance to get out and do some of the things you need to do. Whether is means just a trip to the bank, a movie or a vacation, we can help you relax by knowing your loved one is cared for well while you are away.
Light Housekeeping
When you are at this point in life, house work can become a real nuisance. So stop doing it!
Let one of our caregivers take over this job. Our staff will clean the areas used by you and we will clean up after meals. We don't tell you "we don't do windows." Axiom Home Health Incis licensed, bonded and insured.

Insurance
Private Duty Home Health care accepts certain private and long term care insurance. Policies vary from person to person so we cannot say for certain which companies will cover our services.
What we can say, is that we are happy to assist you through the insurance process to find out exactly what benefits are available to the client in regards to our services.
Medicare/Medicaid
Private Duty Home Health care does not provide Medicare or Medicaid services. If you have Medicare or Medicaid and they cover home healthcare services, we will recommend you use a home health company that accepts Medicare and Medicaid. We want what is best for you, not only in regards to health and wellness, but financially as well.
If you would like additional nursing care above and beyond what Medicare or Medicaid will cover, and would like to prevent or delay an admission to a hospital or extended care facility by having a nurse come into your home to care for you, Private Duty Home Healthcare may be able to assist. We would coordinate our care with your current home healthcare provider and personal physician to ensure that all your needs are met. This additional care may be covered under a Medicare supplemental policy or separate Long Term Care policy, or can be provided for private pay. Private Duty Home Healthcare is more than willing to do the insurance investigative work for you to determine what the provisions of your policy are in regards to our services. We can assist clients with the claim process for reimbursement from a Medicare supplement policy, although clients under these policies must pay Private Duty Home Healthcare directly.
